Physical Examination and Immunization Schedule


 

We are dedicated to helping your child(ren) grow up as healthy as possible.

Because we believe in the protective health benefits of immunizations, we are a vaccinating clinic and follow the recommended guidelines of the American Academy of Pediatrics.

The following Well Child Exam and Immunization Schedule will help you anticipate the visits and screenings that your child(ren) will need throughout their childhood:

First newborn visit, 2-5 Days after birth:

We will review your baby’s birth weight and birth history with you.
Please bring any papers or results from the hospital.
We will do a weight check and Physical Examination as well as discuss any concerns or feeding issues.

1 Week – 2 Week visit

  • Physical Examination and weight/feeding check
  • Obtain 2nd Newborn Screen/PKU (1st one done at birth)

1 Month visit

2 Month visit

  • Physical Examination and Immunizations - Pentacel (DTaP, Polio, HiB), Prevnar 13 and Rotateq
  • Maternal post-partum depression screening
  • Developmental surveillance / screening

4 Month visit

  • Physical Examination and Immunizations - Pentacel (DTaP, Polio, HiB), Prevnar 13 and Rotateq
  • Maternal post-partum depression screening
  • Developmental surveillance / screening

6 Month visit

  • Physical Examination and Immunizations -Pentacel (DTaP, Polio, Hib), Prevnar 13, Rotateq and Hep B as well as 1st Influenza (Flu) vaccine (depending on the time of the year)
  • Maternal depression screening, if needed
  • Developmental surveillance / screening

9 Month visit

  • Physical Examination only. Vaccine “catch-ups”
  • Ages and Stages  9 month Developmental screening
  • Hemoglobin, Lead screen and/or lead testing

12 Month visit

  • Physical Examination and Immunizations - MMR (measles, mumps, rubella), Varivax (chicken pox), Hep A, Prevnar 13
  • Developmental surveillance / screening
  • Instrument-based Vision testing
  • Hemoglobin, Lead screen and/or lead testing
  • Fluoride varnish
  • TB (tuberculosis) screen and/or PPD test

15 Month visit

  • Physical Examination and Immunizations - HIB, Prevnar 13 (if not given at the 12 months visit)
  • Developmental surveillance / screening
  • Fluoride varnish

18 Month visit

  • Physical Examination and Immunizations - DTaP, Hep A
  • Ages and Stages 18 month Developmental screening
  • MCHAT Autism screening
  • Fluoride varnish

2 Year (24 Month) visit

  • Physical Examination, Lead, Hemoglobin
  • Ages and Stages 24 month Developmental screening
  • MCHAT Autism screening
  • Instrument-based Vision testing
  • Fluoride varnish
  • Dyslipidemia screening if at risk

2 ½ Year (30 Month) visit

  • Physical Examination
  • Ages and Stages 30 month Developmental screening

3 Year (36 Month) visit

  • Physical Examination
  • Developmental surveillance / screening
  • First Blood Pressure (BP) check
  • Instrument-based Vision testing
  • Fluoride varnish

4 and 5 Year visits

  • Physical Examination 
  • Hearing and vision screening
  • Instrument-based Vision testing performed if unable to cooperate with wall eye chart
  • Immunizations - DTaP, Polio, MMR (measles, mumps, rubella), Varivax (chicken pox)
    *Vaccines can be given at the four or five year Well Child visit at the discretion of the parent(s) and provider. These vaccines must be given before the child enters kindergarten
  • Developmental surveillance / screening
  • TB (tuberculosis) screen and/or PPD test

6, 7 and 8 Year visits

  • Physical Examination and hearing/vision
  • Wellness, diet and exercise promotion

9 – 10 Year visits

  • Physical Examination and hearing/vision
  • Dyslipidemia (cholesterol) screening

11 and 12 years visit

  • Physical Examination and hearing/vision
  • Immunizations - Tdap booster, Meningococcal and HPV (Human papillomavirus vaccine)
  • Dyslipidemia screening, if not previously obtained
  • Depression screening

13 and 14 Year visits

  • Physical Examination and hearing/vision
  • Depression screening
  • CRAFFT screen/alcohol and drug use assessment

15 and 16 Year visits

  • Physical Examination and hearing/vision
  • Depression screening
  • CRAFFT screen/alcohol and drug use assessment

17+ Year visits

  • Physical Examination and hearing/vision
  • Dyslipidemia screening
  • Depression screening
  • CRAFFT screen/alcohol and drug use assessment
